Army form W. 5080, sent by the Officer in Charge of Records, Durham Light Infantry, York, requesting details of the living relatives of a deceased soldier, Private John H. Wardell. The form had to be certified by a minister or magistrate, and returned before the plaque and scroll commemorating the soldier could be issued. Part of a collection relating to John H. Wardell.
